Gout,is the result of deposition of urate
crystals in connective tissues as a result of
faulty metabolism causing a build up of
high urate levels in the blood.It affects peripheral joints,
causes intense pain, redness and swelling, and more often than
not involves the metatarsophalangeal joint of the big toe.The
instep, ankle, knee, wrist and elbow are common sites.
When the levels of uric acid go beyond a
certain concentration urate crystals are formed and deposited
in synovial fluid and with the progression of gout, urate
crystals form in subcutaneous tissues and these formations of
small white nodules or tophi are visible through the skin.
There is another type of gout known as pseudo
gout which is due to the formation of calcium phosphate
dehydrate crystals.Both types have the same effect.
The cause of the metabolic defect is
unknown,although some researchers consider gout to be an
inherited enzymatic defect.
It's onset can be sudden, and may be triggered
by overindulgence in food or alcohol.Other factors may be
involved in the initial onset, minor trauma, emotional stress,
infection, surgery,or vascular occulsion.
Kidney function plays a role in the pathology
of gout because most uric acid is eliminated through the
kidneys.Renal stones are many times more common in people with
primary gout, and large stones can cause progressive renal
failure.
Medically the condition may be treated
with colchicine, indomethacin and phenylbutazone.
Hydrocortisone may be injected into the joint to relieve the
pain. The patient is given a low purine diet to follow,and high
fluid intake.
Pseudogout: Most people with this condition
have no evidence of underlying disease, however a metabolic
disorder may be found in some people eg. hyperthyroidism,
hyperparathyroidism,or hemochromatosis.
So what are purines? They occur as end products
in the digestion of certain proteins, some are synthesized in
the body. They can be found in certain medications, anchovies,
sardines, sweetbreads, liver, kidneys, legumes and poultry. The
foods lowest in purines include vegetables, eggs, fruit,
cheese,and nuts.

chicken,peas, crab, salmon ,

Low purine foods:
Carbonated beverages,chocolate, cocoa, fruit juices, , white
bread, crackers,
cornbread, cereals and cereal products, corn, macaroni,
noodles, rice, tapioca,

sugar, sweets, vegetables of all kinds except those

The above list of low purine
foods is certainly not a list to be used as a
reference for a gouty patient to follow.
The ideal diet for a patient with gout would
